Care Manager of the Month: Tammy Potter

Every month, NCI profiles a local care manager to help the community learn more about the valuable role they play in our region’s healthcare system.

The “Care Manager of the Month” for August 2017 is Tammy Potter!

Tammy has worked in the healthcare field for nearly 30 years and currently serves as a Chronic Care Manager and Depression Care Manager at the Carthage Family Health Center, one of Carthage Area Hospital’s outpatient locations.

To get to know her better, we sat down with Tammy and asked her a few questions about herself and her job:

Tammy Potter is our “Care Manager of the Month” for August 2017!

Q1: Where are you from?

“I was born and raised in Upstate New York. I moved away for a few years but decided to make my home in this area.”

Q2: How long have you worked at the Carthage Family Health Center?

“I started working as the Chronic Care Manager/Depression Care Manager at Carthage Family Health Center in November 2016. Prior to that, I worked as an ICU/PCU nurse for several years at a local hospital. I’ve been in the healthcare field for nearly 30 years now.”

Q3: What do you like most about Care Management?

“I love the individual, one-on-one contact with my patients. When working in the nursing unit, you don’t always know what happens after the patient is discharged. Care Management allows you to follow-up and continue caring for the patient to see the effect of their efforts. This program impacts the patient’s life with a whole-person approach. I enjoy the prevention, health maintenance, and social aspects of care management quite a bit. My Chronic Care patients really like the interaction and I know it’s making a difference in their lives.”

Q4: What do you like to do outside of work?

“I enjoy being active and like to take part in whatever Upstate New York has to offer – from hiking or kayaking in Old Forge, to tubing and attempting to ski at Dry Hill. I also enjoy spending as much time as I can with my two boys.”

Q5: What’s your favorite movie?

“I’m a big Gerard Butler fan. My favorite movie is ‘P.S. I Love You’.”

Q6: Is there anything you want others to know about you or Care Management?

“I’d like others to know that care management is an amazing program. Not only do you get to make a meaningful impact with patients, but it also provides you with opportunities to get to know and interact with other providers, nurses, community resources, and other members of the community.”
