Erika Flint
Erika began serving as the Executive Director of the Fort Drum Regional Health Planning Organization and NCI Director in December of 2016. She continues to lead the organization’s mission to analyze the regional healthcare system, identify gaps, and leverage resources to support access to high-quality healthcare services for the Soldiers, families, veterans, and civilian residents of the Fort Drum region.

Prior to her role as Executive Director, Erika joined the team at the North Country Initiative (NCI) as DSRIP Director in December 2015. Her primary role was to provide oversight and coordination of the development and implementation of all projects within the Delivery System Reform Incentive Payment Program (DSRIP). DSRIP brought approximately $73 million to the region over a five-year period for the implementation of clinically-driven projects across Performing Provider System (PPS) aimed to improve quality, reduce costs, and improve the patient experience.

Erika's career in public service includes her role in two community 501(c)(3) non-profit organizations. She served as Executive Director of the Watertown Urban Mission, providing oversight of seven major programs serving over 35,000 individuals in need annually. Prior to the Watertown Urban Mission, Erika served as Assistant Director of Residential Services at the Children's Home of Jefferson County.

Erika has a Bachelor of Arts from SUNY Oswego and a Master of Business Administration from the University of Phoenix, with a concentration in Healthcare Administration. She currently serves on the North Country Regional Economic Development Council, Chair of Safe Schools Endeavor, and is a member of the Watertown Noon Rotary Club. She is also a past Board member of the Watertown Local Development Corporation, Jefferson County Community Services, WPBS-TV, North Country Family Health Center, and Samaritan Health Auxiliary, as well as a past member of the Greater Watertown Jaycees and the Northern New York Community Foundation’s LEAD Council. Erika is a graduate of the Greater Watertown Chamber of Commerce’s Jefferson Leadership Institute, and is a recipient of NNY Business Magazine’s 20 Under 40 Award, the ATHENA Leadership Award®, and the 2015 Successful Business Award in Social Services.

Erika resides in Dexter, NY, with her husband Travis and their two sons.
Joanna Loomis
Joanna Loomis joined the North Country Initiative (NCI) team in 2018 as Director of Provider Strategy and Transformation, and in 2021, she assumed the role of NCI Deputy Director.
​
Previously, she served as the Executive Director of the Watertown Urban Mission, where she provided leadership and oversight to its seven major programs, which provide basic needs and empowerment to individuals and families in need in greater Jefferson County. She has also served as the Mission’s development director and critical needs coordinator, and as a child protective services caseworker with the Jefferson County Department of Social Services.

Joanna has a MA in social and public policy from SUNY Empire State College and a BA in philosophy from St. Lawrence University. After completing her undergraduate degree, she lived in New Delhi, India, where she taught English in a public school as part of a Fulbright grant.
​
Joanna serves on the boards of the North Country Prenatal-Perinatal Council and Watertown Noon Rotary; is a member of the Northern New York Community Foundation’s LEAD Council; is a graduate of the Jefferson Leadership Institute; and is a co-chair of the region's Bridges Out of Poverty Steering Committee. She previously served on the boards of the North Country Family Health Center and the North Country Council of Social Agencies. Joanna is a Lean Six Sigma Green Belt, a trained "Bridges Out of Poverty" and "Bridges to Health & Healthcare" trainer, and is certified through the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home Content Expert (PCMH-CCE).

Joanna lives in Henderson with her husband, two children, and a terrier mix.
Lindsay Baldwin
Lindsay Baldwin serves as Chief Financial Officer (CFO) of FDRHPO and NCI. Prior to her role as CFO, she served as Deputy Director of the North Country Initiative (NCI). Lindsay joined the team in August 2015 as Director of Finance, a role in which she managed the Delivery System Reform Incentive Payment (DSRIP) program financial implementation plan and funds flow. In 2017, Lindsay also managed the implementation of the DSRIP program for NCI and assists with management of the regional Accountable Care Organization (ACO) and Independent Practice Association (IPA).

Lindsay began her studies at Jefferson Community College and completed her Bachelor of Science in Applied Economics and Management at Cornell University. Over the course of her professional career, Lindsay has gained extensive experience in financial analysis. As a former Financial Analyst, she supported the U.S. Navy while employed as a government subcontractor to manage budgets for fleet support. Lindsay is also Lean/Six Sigma Green Belt certified, having led multiple process improvement projects and events, and currently serves on the North Country Family Health Center Board of Directors.

Lindsay resides in Adams, New York with her husband Trevor and their two daughters.
Heather Petrie
Heather Petrie joined NCI in February 2022 following her role as a Community Health Worker at North Country Family Health Center (NCFHC). Her work has revolved around addressing patient barriers to care, including social determinants of health. She has extensive work in care gap closures, EMR documentation to include ICD coding for quality compliance, working with HealtheConnections, and has been instrumental in implementing transitional care management.
​
Heather holds an associate degree in Humanities and Social Psychology and a bachelor’s degree in Human Development. She is also trained in Bridges Out of Poverty, Health Literacy, and Trauma-Informed Care, and is a Bridges Out of Poverty certified trainer.
​
She is certified through the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home Content Expert (PCMH-CCE)and is a Certified Healthcare Technology Specialist in Practice Workflow and Information Management Redesign.

Originally from Detroit, Michigan, Heather currently lives in Adams Center, NY. In her spare time, she likes to read and volunteer in the community.
Heather Sanchez
Heather serves as Office and Program Administrator for both FDRHPO and NCI. Before her current role, Heather was the Administrative Coordinator for North Country Initiative (NCI).
Heather has provided overall administrative and program support to the management team and FDRHPO and NCI programs since 2014. In her current position, she provides support to the FDRHPO Executive Director/NCI Director, Chief Financial Officer, and HIT Director/Security and Compliance Officer including support with boards and committee meetings and financial and compliance-related tasks.
She earned an A.S. degree in Business Administration in 2007 and after completing her internship with FDRHPO, she completed her A.A.S. degree in Office Technology – Medical in 2014 from Jefferson Community College (JCC).
Heather resides in Calcium with her husband, Alfredo, who is a Retired US Army Major working at Fort Drum, her daughter, Elena who is currently attending JCC Nursing Program and two dogs, Rex and Saylor.
Ian Francis
Ian joined FDRHPO as a Practice Transformation Specialist in 2017. He works closely with clinical and community-based partners in the region to provide Patient-Centered Medical Home recognition support, as well as support to improve performance in value-based arrangements. Ian excels in transforming payor and third-party data into actionable reports that support connecting patients in the region with recommended preventive services, such as annual wellness visits, cancer screenings, chronic disease management, immunizations, and more.
​
He holds a bachelor's degree in Healthcare Administration and is certified through the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home Content Expert (PCMH-CCE). Ian previously worked with the Fort Drum Medical Department Activities as the Practice Manager for their Primary Care, Dermatology, Pain Management clinics and Fort Drum Emergency Medical Services. While serving in the military, he gained both work and management experience in Rehabilitation, Public Health, Emergency Departments, Emergency Medical Services, Healthcare Recruiting, and various medical operations in multiple countries.

He has been married for almost 30 years to his wife, Leslie.
Tracy Hart
Tracy works as a Practice Transformation Specialist for FDRHPO. She works with partners in the tri-county region, providing support for value-based initiatives and person-centered care. She works with practices to optimize their electronic medical records and provides support to proactively reach out to patients to close care gaps. Tracy also helps the region’s behavioral health practices develop and implement innovative strategies to increase quality, integrate behavioral and physical care, and develop value-based reimbursement strategies.
​
Tracy has worked in the nonprofit sector for most of her career, as an IT Coordinator and Administrative Assistant. She also has worked in the medical field as a certified nursing assistant and psych tech.
​
She is certified through the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home Content Expert (PCMH-CCE) and is a Certified Healthcare Technology Specialist in Practice Workflow and Information Management Redesign.

She is a native of the North Country and enjoys spending time with her family and friends.
Dr. Steven Lyndaker
As our part-time Medical Director, Dr. Steven Lyndaker provides clinical leadership and medical expertise and serves as an ongoing liaison for our Accountable Care Organization (ACO) and Independent Physician Association (IPA) partners, subcommittee leaders, and other healthcare organizations in our region.
​
He is an internist/partner at Lowville Medical Associates and an Associate Clinical Professor of Medicine at the University of Rochester School of Medicine, teaching residents from the program in his office several times a year.
​
Dr. Lyndaker enthusiastically supports FDRHPO’s mission to improve healthcare delivery to our region. He lives in Lowville, NY, with his wife and five children.
