What is DSRIP?

"DSRIP" stands for Delivery System Reform Incentive Payment Program

On April 14, 2014, the State of New York (the State) and the Centers for Medicare and Medicaid Services (CMS) reached an agreement on a groundbreaking waiver that allowed the State to invest $8 billion of the $17.1 billion in federal savings generated by the Medicaid Redesign Team (MRT) reforms. The waiver allowed for comprehensive Medicaid delivery and payment reform primarily through the DSRIP program.

The $6.42 billion DSRIP program provided incentives for Medicaid providers to create and sustain an integrated, high-performing healthcare delivery system that can effectively and efficiently meet the needs of Medicaid beneficiaries and low-income, uninsured individuals in their local communities by improving the quality of care, improving the health of populations and reducing costs.

The DSRIP goal: To reduce avoidable hospital use by 25 percent over the five-year period.

To accomplish this, 25 Performing Provider Systems (PPS) were established statewide as partnerships between hospitals, primary care, Behavioral Health, EMS, Public Health, and other providers across the healthcare continuum and community-based organizations. Each PPS chose specific projects to best meet their communities’ needs. PPS's were composed of regionally collaborating providers tasked with implementing the projects spanning three domains: system transformation, clinical improvement and population health improvement (New York State’s Prevention Agenda).

North Country Initiative (NCI) was the PPS for the Tug Hill/Seaway Valley Region, which includes Jefferson, Lewis and St. Lawrence counties.

Five years later...

As a result of the collective efforts of all NCI partners and providers, we met and exceeded the DSRIP goals as follows:

25% Reduction in Potentially Preventable Admissions (PPA) through Measurement Year (MY) 4

27.3% Reduction in Potentially Preventable Readmissions (PPR) through Measurement Year (MY) 4

How did we do it?

Our overall impact can be attributed to the individual successes of each project we chose to undertake. Each of those projects, as well as real-life testimonials from patients, partners and providers, are showcased in NCI’s DSRIP overview publication “5 Years of High 5s.” We invite you to peruse the pages of this publication and read how DSRIP truly transformed the healthcare delivery system in our region.
