Care Manager of the Month: Elizabeth Kelley

Every month, NCI profiles a local care manager to help the community learn more about the valuable role they play in our region’s healthcare system.

The “Care Manager of the Month” for September 2017 is Elizabeth Kelley!

Elizabeth has worked for Claxton-Hepburn Medical Center for five years and currently serves as the Chronic Care Coordinator for the Claxton-Hepburn Health Centers. She provides care management services to all the health center locations.

To get to know her better, we spoke with Elizabeth and asked her a few questions about herself and her job:

Elizabeth Kelley is our “Care Manager of the Month” for September 2017!

Q1: Where are you from?

“I was born and raised in Heuvelton, NY. I graduated from Heuvelton Central School and obtained my Bachelor of Science Degree in Nursing from SUNY Canton.”

Q2: How long have you worked at Claxton-Hepburn?

“I’ve worked at Claxton-Hepburn Medical Center for five years. I started off working as a floor nurse in the Medical-Surgical unit and later worked as the critical care float. I’ve been working in the Care Coordinator role at the Health Clinics since November 2016.”

Q3: What do you like most about Care Management?

“I really enjoy the individual care with all the patients. When you work in the hospital, there is not a lot of time spent with patients, whereas now I am able to follow up and work directly with the patients on their goals. I’m able to spend more time with the patients and see the impact I have on their lifestyle versus only seeing what happens while they are in the hospital.”

Q4: What do you like to do outside of work?

“I love spending time with friends and family. I have a 1-year-old son and a 4-year-old step-daughter. I enjoy reading and doing outside activities such as four-wheeling and snowmobiling.”

Q5: What’s your favorite movie?

“Sweet Home Alabama”

Q6: Is there anything you want others to know about you or Care Management?

“Care management is a program that helps close the loop and make an impact on the patient you serve. You can really see the changes occurring in that person’s life that you don’t usually see working as a nurse in the hospital.”
