Care Manager of the Month: Netty Diaz

Every month, NCI profiles a local care manager to help the community learn more about the valuable role they play in our region’s healthcare system.

The “Care Manager of the Month” for November 2017 is Netty Diaz!

Netty has worked for St. Lawrence County Community Services as a Case Manager for the past eleven years. To get to know her better, we spoke with Netty and asked her a few questions about herself and her job:

Our “Care Manager of the Month” for November 2017 is Netty Diaz.

Q1: Where are you from?

“I am originally from Puerto Rico. I moved to St. Lawrence County approximately 13 years ago.”

Q2: How long have you worked at the St. Lawrence Psychiatric Center?

“I have worked for St. Lawrence County Community Services for the past eleven years. I currently work as a Case Worker with the adult population. Prior to working at SLC-CS, I worked as a Medicaid Services Coordinator at Cerebral Palsy of the North Country.”

Q3: What do you like most about Care Management?

“Providing community and home-based services. I enjoy seeing the progress that individuals and their families make. I also enjoy the fact that no two days are the same. I thrive off constant change. Seeing individuals in their own environment is invaluable.”

Q4: What do you like to do outside of work?

“I enjoy traveling. I also like spending time with my family. I have two sons — Cody, who is 28-years-old and lives in Connecticut, and Shane, who is 22-years-old and lives in Rochester (Nazareth).”

Q5: What’s your favorite movie?

“I like ‘The Notebook’ and ‘The Godfather’.”

Q6: Is there anything you want others to know about you or Care Management?

“I have been in and out of the private sector since 1988 doing some form of case management. I’ve covered urban, suburban and now rural.”
